Born in Toronto, ON, but grew up in Winston-Salem, NC.
His father, a minor league hockey player nicknamed "Crazy John", was very demanding when he was growing up and abused the younger O’Sullivan both verbally and physically when he had an off-game. He was physically beaten every day. The abuse reached a point where Patrick O’Sullivan pressed charges against his father and filed a restraining order against him while having a highly successful career in the OHL.
At the 2003 NHL draft which saw him freefall as a projected top-five talent way down to 56th overalll he was flanked by two security guards to potentially protect him from his father.
He later detailed the abuse in a book, "Breaking Away: A Harrowing True Story of Resilience, Courage and Triumph". In one of the passages he wrote: "I’d be looking at an hour or two or more of my father’s conditioning program, running the steps in the arena stands like a hamster on a treadmill or chasing after the van for two or three miles. If he didn’t think that was toughening me up, he’d slap me around. Every year he was ramping it up: slap in the face when I was eight; a slap with more force and a kick in the ass when I was nine; a punch when I was 10; a big right hook on my jaw and a kick in the gut or ribs until I was gasping when I was eleven, twelve and thirteen."
Tremendous hands and a good shot. Explosive scorer and a very creative forward. Effective at all three positions up front.
Average size and strength. Not a great player without the puck and weaker defensively.
Signed a contract before the 2012/13 season with Finnish club HIFK Helsinki. His stint there was shortlived as he played his last game in Finland in October 2012 before requesting to be released by the club. He ended his career and went on to live in Naples, Florida.

Lors de la draft NHL en 2003, qui l'a vu chuter du top-5 projeté jusqu'au 56e rang au classement général, il était flanqué de deux gardes de sécurité pour le protéger potentiellement de son père.
Plus tard, il a détaillé les abus dans un livre titré "Breaking Away : histoire vraie et poignante de résilience, de courage et de triomphe". Dans l'un des passages, il écrit : "Pendant une heure ou deux ou plus du programme de conditionnement physique établi par mon père, je courais dans les marches de la patinoire comme un hamster sur un tapis roulant ou je courais derrière la camionnette familiale sur deux ou trois miles. S'il pensait que cela ne m'endurcissait pas assez, il me giflait. Chaque année cela montait crescendo : une gifle au visage à 8 ans ; une plus forte et un coup de pied aux fesses à 9 ans ; un coup de poing à 10 ; un gros crochet droit sur la mâchoire et un coup de pied dans le ventre ou dans les côtes jusqu'à ce que j'en ai le souffle coupé quand j'avais 11, 12 et 13 ans."
O'Sullivan avait d'incroyables mains et un bon lancer. Buteur explosif et attaquant très créatif, efficace aux trois positions offensives.
Taille et force moyennes (1m80, 86 kg). Faible défensivement et dans le jeu sans paley.
Il a signé un contrat avant la saison 2012/13 pour le HIFK Helsinki, mais son séjour en Finlande fut bref puisqu'il demanda à être libéré par le club en octobre 2012. Il mit fin à sa carrière et s'installa à Naples, en Floride.
